ABSTRACT:
The Karindas Grading System (KGS): Integrating Regenerative Medicine for a Comprehensive Understanding of Cancer Origins
In the field of oncohistopathology and oncobiopathology, grading systems play a crucial role in assessing tumor characteristics, guiding prognostic evaluations, and informing therapeutic decisions. However, the existing grading systems have limitations in interpretation, application, and uniformity. To address these challenges, we present the Karindas Grading System (KGS), a novel and comprehensive approach that integrates regenerative medicine concepts to revolutionize our understanding of cancer origins.
This presentation begins with an overview of the current grading systems used in oncohistopathology and oncobiopathology. These systems often rely on three-tier scales based on histologic characteristics, including parameters like mitotic activity and tumor necrosis. While practical, these systems have limited boundaries and lack the efficiency required for effective clinical case management and research studies.
We then introduce the development of the Karindas Grading System (KGS) in the context of regenerative medicine. The KGS aims to provide a more holistic assessment of tumor cells by examining their maturation, differentiation, and alterations, as well as evaluating their cellular activities and organization. By incorporating regenerative medicine concepts, the KGS offers a deeper understanding of phenotypic and genotypic descriptions in vivo, enabling precision diagnostics, prognostic evaluation, and tailored therapies.
The role of the KGS extends beyond individual tumor cells to assess tissue identities within the context of cancer progression. It offers insights into the dynamic interactions between cancer cells and their microenvironment, with implications for regenerative medicine research and the development of innovative therapy approaches.
In clinical practice, the KGS has significant applications. It can be integrated into tumor staging and prognostic evaluation, providing clinicians with a more comprehensive and informative assessment of patient cases. Additionally, the KGS enhances personalized treatment approaches, leveraging regenerative medicine techniques to optimize therapeutic decision-making for improved patient outcomes. Case examples showcase the practical application of the KGS, highlighting its potential in guiding clinical management.
Furthermore, the KGS unveils the obscure corners of oncogenesis, contributing to a better understanding of the underlying processes. It aids in identifying key factors and mechanisms involved in cancer development, paving the way for targeted interventions and regenerative medicine-based therapies.

ABSTRACT:
Adipose-derived Mesenchymal Stem Cells in the Treatment of Degenerative Spine Conditions: Protocol and personal experience with DDD
Lumbar degenerative disc disease (DDD) poses an ongoing challenge as far as treatment options and alternatives, especially when considering younger patients. Over 80% of the adult population presents with one or more episodes of ongoing-progressive low-back pain (LBP). The primary cause is associated with degeneration of the intervertebral disc and which is triggered by a decrease in the nucleus pulposus cell population, as evidenced in histological studies. Definitely, in the presence of a “black disc” without profusion and neurological compromise, microsurgery or even fusion surgery should not be contemplated. Numerous percutaneous techniques have been propagated as the proper way to treat this condition throughout the literature in the past years. They have been primarily focused on the treatment of the pain generated by the involved disc and the subsequent segmental insufficiency, without addressing the degeneration of the disc and for this have had limited success and remain as pain management tools. Some significant trials in the past (i.e. Chondrocyte transplantation trial) and the increasing recent research and achievements with more biological strategies as far as tissue regeneration has motivated the development of a new treatment concept initially applicable to the lumbar spine which will be presented and discussed.
Advancements have led to a significant improvement in the understanding of the cell environment and tissue transplantation at a molecular, cellular and immunobiological level. Adipose tissue has already become a central source of clinical and research work involving adipose tissue-derived progenitor cells. Endothelial and mesenchymal stem cells derived from adipose tissue are being considered and used in an array of clinical conditions and seem to have clear therapeutic benefits for many disease conditions including those affecting bone, cartilage and muscle. The use of an accessible source with abundant cells which have a high potential for regeneration clearly is superior in comparison to the chondrocyte option for the lumbar disc. Mesenchymal cells have a high self-renewal capacity and a potential for multi-lineage differentiation. For this, adipose tissue-derived MSCs (ADMSCs) are optimal candidates for tissue regeneration and can be obtained from the patient in a one-step procedure treatment.
